Place cocoa powder, sugar and rum (optional) in a mug. Peel a strip of rind from orange using a vegetable peeler and squeeze 1 tbs juice. Thread peel onto a skewer and set aside. Heat milk with juice to boiling point. Pour into mug, using the orange skewer to whisk vigorously until combined. Dust with extra cocoa and serve immediately.
